This Order Form is subject to and governed by the terms and conditions set out in the online Terms Of Service (TOS) entered into between the Customer identified above and Qapita (as such term is defined in the TOS).

Your QapMap Starter Plan Subscription Term with Qapita will commence on Start Date, the date when number of stakeholders recorded on QapMap exceeds 24. Recurring charges of USD 480.00 (excluding any applicable sales taxes) will be payable annually commencing on the Start Date and each anniversary thereafter. You are required to comply with the conditions of the QapMap Starter Plan and we (Qapita) reserve the right to upgrade the pricing to correspond with the appropriate QapMap subscription plan in subsequent subscription period(s).
